Before You Schedule
I understand that intake paperwork and consents are required.
Once your online appointment request is approved, you will be sent a list of intake paperwork (to your email and phone number provided). Completion of this paperwork is required within 48hr of receiving the link. Failure to complete the intake paperwork may result in your appointment being rescheduled or cancelled.
I understand that this is a non-insurance accepting clinic.
Services are self pay and payment is due at time of service. If requested, a superbill can be provided so you can request insurance reimbursement through your carrier.
Physical Location Requirements
Patient's must be located within the state of a practitioner being licensed. Lindsay is licensed to treat both Maine and Florida residents. Lauren is licensed to treat Maine residents.
Attention Medicare Beneficiaries:
Even though we are not an insurance accepting clinic, Medicare will not allow self-pay patients to see providers who are "opted in" (meaning they may treat Medicare patients at other locations).
Lindsay is a provider who can see Medicare patients, Lauren is not. Medicare beneficiaries who schedule online with Lauren will be rescheduled with Lindsay at a different date and time.
