Be sure to choose whether or not you are a NEW or RETURNING patient. This is important for charting purposes.
Parents scheduling for their children: the appointment request needs to be in the CHILD'S name and birthday.
Once the appointment is requested, we will send along a confirmation and the required consents and intake paperwork. If you do not receive a link to complete consents (via email or phone) within 48 hours, please contact the clinic (don't forget to check your spam folder).
If we do not receive the intake paperwork within 3 days you will be contacted by the office. If we do not connect with you, and no consents are received, your appointment may be cancelled and given to someone else on the wait list.
