Credit/Debit, Cash, Check, HSA/FSA accepted. Superbill can be provided upon request for insurance reimbursement purposes, if applicable. Payment is due at time of service.
Membership
If you are not looking for medical weight loss or hormone therapy, you can enroll in membership for a one-time enrollment fee of $129, followed by $79 per month. Your medical visits are covered—no surprise fees.
This can include your follow-up visits for weight loss, hormone therapy, primary care, chronic care, and urgent care, plus you receive priority scheduling, concierge access, and member discounts.

Medical Weight Loss
The initial visit is $199, and this includes the enrollment fee and the comprehensive visit, where we will evaluate your history, goals, and determine safe treatment options (along with diet and exercise recommendations). Subsequent visits are $99 per visit or (if enrolling in membership) are $79 per month. Weight loss patients are seen monthly for accountability and to determine the effectiveness of the treatment plan and for any necessary adjustments. $299 includes your first two appointments. The first appointment is a comprehensive discussion regarding your history and symptoms, an order for labs to be drawn using insurance benwhich may be done at your lab of choice, and the the second visit is for review of labs and the development of a treatment plan. Follow-up appointments are $99 for non-members and may be every 1, 3 or 6 months. If labs or prescription costs are a concern, we have options to assist. Membership is available if desired.

Urgent Care/ Sick Visit
Cost includes appointment only. Prescription sent (if any) will be the patient responsibility (insurance or cash), and can be sent to your pharmacy of choice. In clinic visits that are more complex will be $129 and will include indicated testing such as strep swab, flu/covid testing, urinalysis, x-ray, referrals, etc.

Primary Care Bridge Services
Cost includes an appointment-only visit and one or two conditions to be addressed within 30 minutes. Longer, more complex visits will be charged $129. Prescriptions sent (if any) will be the patient’s responsibility (insurance or cash) and can be sent to your pharmacy of choice. Appointment frequency and recommendations will be reviewed during the visit. Membership welcome.

IV Infusions
Offering multiple different blends of IV hydration with vitamins, hydration, immune support, illness recovery, athletic performance, energy, and chronic disease support. Infusions last 30-60 minutes and clients leave feeling refreshed and energized.

Body Composition Scan
Using the InBody 580 scale you will learn about your particular body composition. The tool analyzes body fat, muscle mass, water weight and visceral fat. This helpful tool is useful for the elderly to identify frailty, for athletes looking to increase strength and muscle mass, and for those on weight loss journeys. Pregnant patients or patients with metal implant devices are not eligible.
Neurotoxin & Dermal Fillers
Offering neurotoxins (Xeomin, Botox, Dysport, Daxxify) for treatment of fine lines and wrinkles, as well as therapeutic effects (migraines, TMJ, excessive sweating). Units depend on the treatment area and will be determined during the consultation.
Dermal fillers are used in the face and lips for symmetry correction, to reduce fine lines and wrinkles, to create structure and framework, as well as to volumize normal loss during aging and weight loss.

Microneedling Session
Microneedling offered with FDA approved SkinPen for the reduction of acne scarring and improvement of fine lines, enlarged pores and overall improvement of skin texture to face and neck.

On Site Lab Services
No insurance or high out-of-pocket expense? We offer a vastly discounted self-pay lab service for our patients using Access Medical Lab. After meeting with the provider and receiving a lab order, you will know your exact out-of-pocket expense before the draw. HSA/FSA eligible. We can also draw labs through Quest, with use of insurance benefits, for $25 draw fee.

Insurance Reimbursement Requests
Many insurance companies will reimburse their members for visits if it is considered a “plan-covered service.” This will generally include most urgent care visits. If this is something you wish to pursue, ask your practitioner to provide you with a superbill at the end of the visit.
